We are here to help. We know that paperwork and insurance can be confusing. Please feel free to call us anytime with even the smallest question.

For all new or returning patients, please complete the patient forms, the notice of privacy act, pick the affected body part or parts, and fill out the corresponding forms. Bring these forms, the prescription from your doctor, your insurance card, your license, and any other documents relating to your treatment. We accept most insurances. Most insurances require a prescription from your doctor to pay for physical therapy services. Some insurances and self-payment do not require a prescription from a doctor. Please call us with your insurance for specific guidelines. We do our best to provide you with your up to date insurance benefits for physical therapy, but please call or check your own insurance so you know what your limitations are.

The state of Connecticut has “Direct Access” for physical therapy. What does that mean? It means that you can see a physical therapist without seeing the doctor first. This may not apply to all insurances, so it is best to check your benefits.

We participate with all the major insurances companies including Medicare and most commercial, managed care insurance plans (HMO, PPO and POS plans) along with worker’s compensation plans. To be sure, give us a call and our friendly staff will help explain your coverage. If you do not have health insurance or other coverage or have exhausted your insurance benefits, we do offer affordable payment plans. Please call for more specific details.

Physicians or other health care profession may suggest a certain facility but know that it is your right as a patient to choose the best fit for your healthcare needs. Our facilities provide one on one individual treatment with a licensed physical therapist for the entire time you are with us. Recovery is individual, and your physical therapist is trained to help you meet your goals.

Our therapists strive to promote a strong patient-therapist relationship. In order that care, be consistent and at the highest quality, we schedule with the same therapist throughout treatment.

What should I bring to my first office visit?2020-12-27T20:38:36+00:00

You will need your insurance card, a photo ID, the physician’s referral if required by your insurance carrier and your co-pay for the visit. If you have imaging studies such as X-rays, MRI or CT scans taken by your doctor, you can bring these in for review by your physical therapist. We would also need a list of medications for our records.

Your first visit with us for the initial evaluation will last between 45 minutes to an hour depending on additional paperwork. Subsequent visits are scheduled for 40 min but may last up to an hour depending on your needs. Your physical therapist will establish a therapy schedule and treatment plan at the first visit.

Loose fitting, comfortable clothing and shoes are appropriate for the initial evaluation so that the therapist can evaluate your movement.

Your initial visit with us will include a comprehensive evaluation by one of our physical therapist. They will review all the information from your medical history and evaluate your condition and develop an effective treatment plan.
